We offer 2 levels of service. The first is providing the complete DIAMONDS BEHAVIOR ENHANCEMENT PROGRAM for challenging behaviors in nursing homes Toolkit and consultative services to collaborate with the facility and mental health teams in establishing the Program. Ongoing scheduled or intermittent on-site or phone consultative services can be arranged after the Program is established upon request. The second level of service is just providing the complete Toolkit with the facility setting up the Program.
The DIAMONDS BEHAVIOR ENHANCEMENT PROGRAM Toolkit includes Program Admission/Discharge criteria, Job Descriptions, Staff Training Program, Behavior Documentation Forms, Off Unit Privileges, Goal/Rewards Program, Life Skills Program, Sample Behavior Contracts , Resident Intimacy Policy , Program Graduation/Off-Unit Transition and more
